By submitting my registration online I am agreeing to all the rules for the tournament, agreeing to abide by them and fully understand that I am agreeing to waive any claims against any persons connected with the N.C.M.A Tournament for any injuries that I may sustain and likewise assume full responsibility for all my actions in connection with said tournament. I completely understand that my participation in said tournament could produce bodily injury or even death and accept those possibilities in order to compete. I understand that if I am under 18 years of age that a parent or legal guardian must submit this form.
Deadline for online registration is June 1st, 2011. Please fill in all information and click submit registration. Registration fees for online registration will be $25.00 for the first event, $5.00 for the second event and $5.00 for each additional event. You may pay with a credit card by simply creating your own PayPal account.
Deadline for mail-in registration is June 15th, 2011. Please print off the form and mail with your check attached. Registration fees for mail-in registration will be $25.00 for the first event, $5.00 for the second event and $5.00 for each additional event.
Registration at the door. Registration at the door will be $40.00 for the first event, $10.00 for each additional event. Registration on the day of the event will be “CASH ONLY”.
Picnic tickets are $10.00 each for children 12 and under and $15.00 each for adults. (At the door $20 and $15.)
T-Shirts are $20.00 each and are “CASH ONLY” if purchased on the day of the tournament.
Spectator Fees are $5.00 per person at the door if not competing and children under 12 are free. Spectator fees are “CASH ONLY”.
